Supervision:
- Supervises and oversees the daily activities of the Case Managers and Social Workers
- Evaluates and provides feedback regarding individual and team performance (EXCEL)
- Recommends corrective/disciplinary actions for staff within scope of responsibility
- Ensures that staff is providing quality service and functional outcomes
- Develop and implement discharge planning activities in conjunction with Physician, family, Nursing and ancillary services for risk patient population
- Offers counseling and direct social work services to assist with meeting psychological needs of patient/family
- Assess and implement appropriate intervention to address identified patient needs and maintain regulatory requirements
- Attends all required departmental and facility meetings
- Assigns committee responsibilities
- Provide oversight and assistance with referrals reporting, discharge status reports, and other reports as needed
- Conducts assessments to determine patient needs and barriers to disease management of continued recovery
- Documents assessment finding in the clinical record according to established standards
- Coordinates and provides orientation to all new personnel within Department
- Participates in QI initiatives and other Hospital programs
- Ensures staff maintains current licensures/annual review/competencies
- Assists with departmental budget development and compliance
- Serves as internal consultant for all issues pertaining to social services/patient/employee advocacy
- Partners with Managers, Clinical Coordinators, and Human Resources to develop and implement recruitment plan/strategies
- Adheres to NMHS/NMMC Policies/Procedures/Guidelines
- Complies with applicable Local/State/Federal policies/procedures/guideline/regulations/laws/statues
- Refers all potential quality care issues to the Rehabilitation Director to facilitate timely follow-up
- Bachelor's Degree in Nursing or Master's Degree in Social Work; required/LCSW; preferred
- Licensed as a Registered Nurse by the Mississippi Board of Nursing; required
- Minimum of 5 years clinical and/or healthcare experience; required
- Knowledge in basic software programs such as Microsoft Word, Excel, and PowerPoint; required
- Excellent organizational and communication (written and verbal) skills; required
